Hereditary conditions as listed in policy for all breeds and specific breeds are subject to special policy limitations and sub-limits.

Cancer coverage up to per incident limits.

Refers to a specifically identifiable accidental injury or illness.  Recurring and/or chronic conditions shall be considered as one incident.

Premiums fully refundable if cancelled within 14 days of initial issuance and no claims have been submitted or paid.

Send the completed claim form and original paid receipts to Claims Department.

Automatic renewal unless written notice of non-renewal is sent.  There is no guaranteed renewal of coverage.  Premium amount may increase or decrease based on claims submitted and age of pet.

Insured may cancel with written notification to insurer.  Insurer may cancel with written notification to insured for non-payment, fraud, change of risk, and violation of policy terms.

Coverage limitations and premium amount may be affected by breed.
